The publication can also be found on the National Institute of Aging website. The publication describes in detail the elements of advanced care planning including hospice and palliative care, medical decisions that are likely to come up at the end of life and living wills.
This is a well written and easy to read article. The authors take sensitive subject material and simplify it with clear actionable plans for use with clients or your own planning. This course is an excellent reference for nurses, clinical social workers and mental health professionals that work with the elderly.
